Women’s Health Support Services Case Manager (Waipahu Clinic)

JOB STATUS:                      Regular full time

JOB SUMMARY:    This position is responsible for providing case management services to women and newborns who receive medical care at the Health Center and to actively participate in special activities of the Community Health Services Department. This includes assessment, planning, and facilitation for options and services to meet an individual’s complex needs. It is expected to work within the program directives set by funding sources within the philosophical framework set by the Board of Directors, and within the existing network of community service providers and natural helpers.

JOB REQUIREMENTS:     Bachelor’s degree in Social Work or a Health/Human services field from an accredited college.  Demonstrate competency in effective writing skills.  Ability to communicate with people, establish rapport with local residents and the Waianae community.  Can work independently with initiative, sound judgment and follow through. Have strong organizational skills.   One-year experience screening and/or assessing substance abuse desired.  Must possess basic counseling skills and have the ability to perform community outreach.  A current Hawaii State Drivers License and daily access to an automobile are required. License is preferred or eligible to take licensing exam.
